Waiver, Release, and Agreement for Supervised Visitation Services

Participant Acknowledgment and Assumption of Risk I, the undersigned parent, legal guardian, or participant, hereby acknowledge that I have voluntarily chosen to utilize the supervised visitation and/or monitored exchange services provided by Cherish Life Family Services. I understand and agree that while Cherish Life Family Services exercises professional oversight, diligence, and care to maintain a safe environment, the inherent risks associated with family disputes and interpersonal interactions cannot be completely eliminated. I knowingly and freely assume all risks, both known and unknown, associated with my participation in these services.

Release of Liability and Hold Harmless Agreement To the fullest extent permitted by law, I release, waive, and forever discharge Cherish Life Family Services, its owners, directors, employees, independent contractors, and agents from any and all liability, claims, demands, injuries, damages, or causes of action arising out of or related to any loss, damage, or injury that may be sustained during, or as a result of, supervised visits, monitored exchanges, or facility use.

Compliance with Program Rules and Court Orders I agree to strictly adhere to all program guidelines, rules of conduct, and directives given by the visitation supervisor at all times. I understand that failure to follow these rules—or any active court orders governing this case—may result in the immediate termination of the visit, suspension of future services, and prompt notification to the court, attorneys, and/or relevant caseworkers.

Acknowledgment of Supervision and Documentation I understand that all visits and exchanges are objectively observed and documented by a trained supervisor from Cherish Life Family Services. I acknowledge that these objective notes and summaries may be provided to the court, legal counsel, or child welfare agencies as required by law, court mandate, or contractual agreement.
